Keep the pyloric stenosis in mind.  I remember at least 3 babies (in the last 10 years) that were vomiting / not gaining that I suggested could be pyloric stenosis, (nurses can't "diagnose" in Canada) that medical professionals didn't diagnose until about 3  or 4 weeks of age (by that point the babies were in a dangerous situation and all had the "old man look".  All kinds of things were suggested in the meantime including, formula, (mothers told the babies were allergic to their milk), bottles, thickened feeds and medications.

No time to research, (Newark / Nestle keeping me incredibly busy) but I believe pyloric stenosis most common in first born male.
