Diane,

In your post about mom with nipple damage, BF and pumping you stated,
"Though her nipples are slightly smaller than a nickel, she says they swell
and turn purple with pumping."

I worked with a mom whose nipple/areolar complex at least doubled in size
when she pumped, tried larger flanges (Medela's large and extra large) and
continued to happpen, reduced suction, too.  What finally helped her to have
pain-free pumping and obtain most milk was the glass flange-she used two at
once (double pumping).  (She wouldn't use manual expression-felt too time
consuming).

Are her nipples currently too large for baby's mouth or perhaps lack of milk
flow from certain nipple openings due to severed nerve endings and ducts
causing baby to suck with more negative pressure?
