I tended to use a fat bed pillow. However, I have also

I also used a pillow, but not a nursing pillow. I used one of the pillows
that has a valley down the middle and is like an egg crate. The valley
helped me to keep my babies laying on their side completely while nursing,
The pillow helped keep my arms from falling asleep, my first two liked to
nurse for very long periods of time. When I would nurse them to sleep
they'd already be on a comfortable, warm surface and I could put them on
the floor or bed without them realizing they weren't in my arms now. This
was the only way I could get them to stay asleep some days, and it also
helped to keep them sleeping on their sides - per the old SIDS
recommendation. I also used it with my most recent baby, but would tilt it
just slightly so she would gradually roll to her back.
