Perhaps offering the baby the opportunity to find the breast himself in a postpartum like situation might do the trick. Just have both bare from the waist up, place baby on tummy on top of mom and let nature do the trick. Also see Susanne Colson's Biological Nurturing (biologicalnurturing.com). I beginning to see moe and more good results of letting baby take control again in a more natural positioning.

Hi everyone any ides to help this situation.

Baby boy-frst baby, term, spontaneous labour, spontaneous birth, no 
analgesia, physiological 3rd stage, skin 2 skin for 90 minutes plus, first BF
at 
15 mins-assisted by midwife to facilitate 3rd stage.
Now 5 weeks and 6 days, BW=3460 gms now weighs 4600gms, feeds 8-12 
times per day, wet nappies 4-6 heavy per day, mustard stools one large at 
least once per day, sleeping 5-6 hours overnight.
Mum has 3mm diameter hole punch lacerations dead centre of nipple-and 
nipple thrush. Baby had oral symptoms now resolving with Nystat. Mum's 
symptoms are also resolving with Nystat and supportive measures.
Nipple damage has been fairly constant in last 3 weeks. I suspect this baby 
has learnt to feed well enough for his own needs but to the detriment of his 
mother's nipples. Question how do we educate him to latch better.
