The only time I've ever seen an irritation (fissure-like)  at the nipple/areolar junction, it was due to the mom holding the baby away from the breast too far, for fear of smothering her during nursing.  It's an easy fix and so would be worth investigating whether she is doing something like this.

I have a question regarding a mom who is due any minute that wishes to try to breastfeed.  It is her fourth and she hasn't nursed any of the others.  She had breast reduction surgery after her second child and says her breasts did get somewhat engorged after that pregnancy.  She has been leaking colostrum.  She stated that she has experienced breast changes with this pregnancy.  Is there a way to predict whether there are enough intact glands and ducts without knowing the details of her surgery??
