When I commented on used for shields, I forgot one.
I have had occasional babies that seem to have a hard time
maintaining latch.  When I try a DSE (digital suck evaluation), I
feel good jaw action and tongue placement almost immediately.
However, if I try a shield, it is immediately pushed out of baby's
mouth, frequently aiming in a funny direction.  This tells me that
baby is doing something "funky" with its tongue.  It would appear
that the finger is just enough firmer that baby is able to maintain
the tongue in an effective position.  This is often a situation
where I will recommend finger feeding and lots of skin-to-skin
cuddling until baby is able to latch directly to the breast more
effectively.
