I agree with Rachel. I would investigate a mother who did not notice a
change in breast fullness/firmness within a few days swallowing.
I think my handout on "best bf practices" states that a mother should see a
noticeable change in the fullness of her breasts and this will correspond to
an increase in swallowing at breast and more wet and dirty diapers to count.
something like that. True engorgment which is when the breasts are very hard
and milk does not release easily is pathologic I think, altho it is common.
The most frequent cases of this in my area are seen in mothers combining
breast and formula feeding. No surprise there.
