I have had both success and failure in using shields.  We rarely use them
now until mom is several days partum, simply due to lack of f/u. If they go
home using a shield, they have an appointment made for them to see us on an
outpatient basis.  About 60-70% no show =,<

Anyway, my experience has been that if you use a small shield on a mom with
nipple diameter of greater than 1.5 cm, you will get nipple abrasions.
Mom's get very sore.  Whenever I use, I always measure and document.

By the way, does anyone use a consent form outlining risks and benefits
when using a shield?
