A mother on my ward had been struggling since her emergency CS for breech,
to get her baby latching reliably.  It went OK for a day, then engorgement
set in and baby went into orbit.  Mother more and more concerned, very
sensitive to all thoughtless comments that came her way, and really
desperate to BF.  Pumping, plenty of milk, as in more than twice what baby
could consume.  Cup feeding to calm baby before offering breast, syringe
feeding at the breast, she also 'discovered' the dancer hand technique all
by herself.  
On the day before she was planning to go home we had a talk.  I watched baby
at breast and for the second time in our acquaintanceship suggested an
ultra-thin nipple shield.  Mother had been reluctant to try it before,
afraid she'd never get rid of it.  I asked, and she has a dog, and she said
it's the type to eat a shield, too, so we decided it was safe.
Baby really settled with the shield in place and fed for a while on the one
side.  Then she looked like she might try the breast alone.  No go.  Moved
to other side, with shield.  I left the room to get us some water to drink
(no, my office doesn't contain cold running water.)  When I came back,
mother was holding her breath, sitting absolutely still, COUNTING as baby
was nursing vigorously without the shield.  'Ninety-three sucks!!!' she said
as I entered the room.  New record, beating her previous one by about
eighty-five.
