I see that no one posted my response to the baby with nipple pain, so I'll
give my answer.  This baby had started solids, and the pain coincided with
the introduction of cereal. In La Leche League sources, this is called
eczema related to a contact dermatitis.  The mother need not have previous
dermatitis or allergy.  Baby doesn't swallow all the food, and food
particles come into contact with the nipple. Solution: a sip of water to
rinse the mouth.
This is in the Womanly Art of Breastfeeding, and also in the Breastfeeding
Answer Book.
I am a La Leche League Leader (on Reserve, but relatively active).  I became
an LC after serving as a LLL Leader. I think all LCs should read the Womanly
Art and keep the Breastfeeding Answer Book on the shelf.  A subscription to
Leaven, the Leaders' newsletter, is not a bad idea.  There have been several
excellent articles there recently, especially one on positioning.
I feel that my LLL background keeps me grounded in the real world that
mothers and babies live in.  La Leche League is our home grown breastfeeding
culture: I sometimes quibble with League, but the information is almost
always rock solid.
