We are becoming ever aware of our language at Oregon WIC and I need
some help with wording questions on screening questionnaires.  These
questionnaires would be given to all pregnant moms who apply for WIC.

There are two questions we need to ask:
1)  How do you plan to feed your baby?
We really want to try to assume BF as the norm, and yet we need to find
out where the mom is at in her decision on feeding method.  This would
be our starting point for education.  Any suggestions for better wording
on this one?

2)  We want to identify potential problems (hx breast surgery, inverted
nipples, etc.).  We want to find out the information, but yet we don't want
to imply that if you have these conditions it will be a "problem".
Suggestions?
