Suggestions I have learned from my patients were to:

1)       cut the tip of the pacifier off, so there is a small hole.  The
suction will not be the same and they will not enjoy it as much.  (I would
imagine that this only works on the one piece pacifiers)

2)       have a ceremonial farewell to the pacifiers!  First, be sure to
collect all the pacifiers!  Then, pick a few, throwing the rest away, and
tie helium filled balloons to them.  Thank them for helping (toddler) for
becoming a big (boy/girl) now.  Kiss (or suck) them bye bye and release!
Maybe they can get a new baby doll to sleep with, or a new truck to play
with.  It is in the PARENTS control!  When the child fusses, you just say
"pacifier flew away", and it no longer is an option.  Get new books!  Read
them to sleep.  Distraction, distraction, distraction!
