Regarding the mother with a 2 cup difference between her two breasts.
My son definitely preferred one breast over the other and it was so
obvious that I never wore anything clingy and one of my bathing suit
cups had to be taken in.  I made a very deliberate effort to encourage
bf on the 'small' side, was very aware of positioning and making nursing
on that side just as comfortable for both of us as the other, etc. etc.
Never made a bit of difference for me, or him.  I think it can be a
combination of factors:  milk flow different, nipple slightly different,
holding babe differently on non-dominant side, etc.  Over time as my
milk supply decreased in general, the lopsidedness became less obvious
and now the only way I know that there still is a difference is because
my son says "it's empty" ;-)   IMO, all in all a small price to pay for
a beautiful healthy bf baby.
