For my private practice, I use Tom Hale's reference in Medications in Mother's
Milk.  I fax it with Tom's Lact-Fax to the OB following faxing my consult
report.  I rarely have an OB that will not prescribe.  I now have the peds in
the office where I work as staff LC precribe for baby and mom as per Dr.
Hale's recommendations and at my suggestion.  Dr. Hale has a website with info
for Lact-Fax or see info in his book.
