I'd guess the nipple damage the 14 mo old's mother experienced is related to
a cause of biting that I've seen:  as the baby grows, he gets longer in both
directions and ends up tucking his chin in order to keep his mouth in the
right place, unless his mother shifts him over time.  He now has to clench
to stay on, because his mouth is "too high", and sometimes he bites.  If she
moves him "down", his head tips back somewhat and the biting and clenching
diminish or stop.  

I saw it again just a few weeks ago; the mother felt an immediate
improvement when she shifted him so that his head was back a bit more.  And
they'd been nursing fine for over 6 months.

So yes, this is something we need in our biting and sore nipples folders!
