Carol, I think in the times I have used something like this, it isn't
actually forcing the baby's mouth open -- instead it is gentle pressure when
the baby starts opening his/her mouth on his/her own.  If the baby is
clamping shut I don't even try, but sometimes they just need a little nudge
to open wider.
