Interesting to see how ignorant of basic nutrition some HCPs can be. Water for a 1 yr old who is not gaining--what is the caloric content of that? And the nutritional value?
An 18 oz serving of Pediasure has 240 calories.  You can get much more calories into a 1 yrs old in 18 oz of mashed sweet potatoes, avocados, pears, apples, green beans, garden peas, red beans, oatmeal, mashed rutabagas, turnips, beets, steamed cabbage, whole grain rice, whole grain pasta, etc etc. Plus it is not just about calories, it is about getting a variety of nutrients. You don't need a nutritionist to develop a eating plan for humans: keep it simple by telling families to eat a variety of colors of foods ( how many people can even name purple, blue, green. orange and yellow foods--and koolaid and jello doesn't count!! Neither does tan !!) , a variety of the parts of the plants ( stems, roots, leaves, seeds and flowers), and as close to the original source of the food as possible. 
And you can certainly offer food tips and advice to the mom. Have her keep a food diary to see what the baby is eating. If eating jarred foods, some have water added, so the nutritional value is diluted. Many moms do not know how to prepare foods, having grown up on chicken nuggets and hamburgers. An excellent peer counselor activity is to do a food demonstration and tasting to help them learn how to introduce solids and prepare healthy foods for toddlers.
Suggestions can be:
Breakfast: cooked whole grain oatmeal with added mashed fruits-pears, peaches, apples. Toast with fruit puree, or cheese if the mom has added cow's milk products.
Morning snack: yogurt, crackers, whole fruit cup into small pieces
Lunch: cut up bits of chicken, steamed and slightly mashed ( baby needs to learn not to slurp food but to start biting and chewing by now) green beans or peas, cooked rice.
Midafternoon snack: same as morning
Dinner: Small bits of the family dinner. It is ok for a 1  yr old to eat pastas, rice, small bits of meat and veggies
 
Avoid fruit juice due to lots of empty calories--everyone should eat fresh fruit and then drink water for hydration.

( I was told to use butter on toast for my 1 yr old who wasn't gaining-MD didn't know margarine and butter has the same caloric value but with a family history of heart disease, we didn't want to expose our children to animal fats. I was told also to feed him small bits of Snickers candy bars and milkshakes made with whole ice cream and whole milk. So we would have a short, fat child with a increased risk of diabetes and heart disease??? Yeah, Doctor, I am really going to follow that advice! NOT!) And that child grow up to be an US soldier and member of the 82nd ABN who is still one of the the smallest men in the Army, but extremely healthy!!
