It could very well be that the LC had done a thorough assessment, history, 
etc and at that 3 day point felt the baby needed supps and the mom a rental 
pump. Perhaps they had a very problematic hospital course, baby had no milk 
transferat the visit, excess wt loss, jaundice, etc. Likely the LC offered 
options and follow-up but the family did not take her up on it. You are 
hearing only the mom's side of the story. I'm sure that is the true way mom 
perceived her visit. Nurses have occasionally reported crazy things mothers 
say I've told them, but fortunately I document carefully my phone 
conversations, consults, etc.

I'm just making the point that perhaps the present situation could have been 
averted with early aggressive interventions. Also this could be a situation 
where mother is not making enough milk due to a physical situation on her 
part, OR it could be a baby problem where baby is not removing enough milk. 
One thing I look for is whether there was a very noticeable increase in 
breast fullness on or about day 3 or 4 (onset of copious milk production). 
If there is a copious increase, it is less likely to be a maternal problem.
