Way to go, Kathy!  Tongue tie is a mid-line defect (as are clefts) and this
little guy is lucky that you could do the frenotomy.  The shield may act as
a sort of palatal obturator, filling in some of the cleft and helping the
baby to seal the oral chamber (a pre-requisite for creating suction).
Feeding is still going to be effortful for the baby, and the one management
suggestion I would make is to keep the mom doing some post-feed pumping so
that the baby can be supplemented with her hind milk when he fatigues (which
he may do too early in the feed to access the desired amount of calories and
to adequately stimulate her supply).
