Just returned from a conference where Kathleen Huggins gave a really
good presentation on nipple problems.  She talked about a Mom with a
sore nipple that, when she used the pump, literally pulled all the skin
off.  It sounded a little like this case, the amount of skin sloughing.
With her mom it was caused from Impetigo, diagnosed by a Dermatologist.
Could this possibly be the case for this Mom?
Just a thought.
