I have a mom who thought she had the stomach flu, but they are now in the
process of confirming e coli. Her 3 mo old baby just had heart surgery, but
otherwise is doing well.  Mom has been prescribed Septra--  co-trimoxazole--
10 days, and it was concluded that she must pump and dump. She was also told
that there was no other drug that she could be given.

In looking this up, it's an approved by the AAP, L3 in Hale. It is a
combination of Trimethoprim (L3, AAP appv'd, no pediatric concerns) and
sulfamethoxazole (L3, AAP not reviewed, use with caution in weakened
infants, premature infants, or neonates with hyperbilirubinemia).  Hale
recommends sulfisoxazole over sulfamethoxazole, but it also has a caution,
for hyperbilirubinemia, G6PD, weakened infants.

How does this apply to this mother and baby? As I read it, normally one
would not hesitate too much to use the med and breastfeed. However, with the
caution........ should we be concerned? I would appreciate input especially
from the physicians on the list.
