I don't think Dr Karp meant to imply that a mother who chooses to nurse
through the night will be that exhausted, in fact he sites a source to
point out that some do quite nicely with it.

However, I wasn't flabbergasted by his statement about exhaustion. I
tried co-sleeping with my babies, and I couldn't do it. Every sigh,
whimper, or kick jarred me awake. I suppose eventually exhaustion would
have taken over, but is that really a good way to co-sleep with a baby?
Wouldn't that be nearly as bad as sleeping intoxicated with a baby? An
exhausted person can fall asleep at the wheel of a car. Could not an
exhausted mother roll onto her child? I was also never able to find a
comfortable position to breastfeed lying down. Instead, my babies slept
nearby in a bassinet, and I got up to nurse them in a recliner near my
bed. I was able to catnap that way, but never deeply sleep. We did
breastfeed more often during the day than at night, and breastfed for 2
and a half years, hardly an early wean.

The point of all this is that different things seem to work for
different families. Swaddling can be helpful. There's no reason that
swaddling has to keep the baby's hands away from his face. In fact, it
can help keep them close so the baby can find them more easily, which is
especially helpful for premature infants.
