Hi All,
Anne, this baby is doing what I see a lot of 36 wkrs at 3 weeks
doing...still learning.  The best thing you can do for this mom is to tell
her to find an IBCLC in her area for a face to face consult.  Trying to do
the "internet consulting" thing is, IMHO, difficult at best, and not good
for anyone with the types of problems this mom is obviously having.  The
suggestion you made about the older nursling NF forst to give the baby hind
milk is good in that the premie will get the hind milk to help grow, but the
preemie need the fore milk for energy.  Also, is this baby getting enough?
WHat is the babys output (diapers)?  The baby may bee "colicky" bc s/he is
getting too much hind milk and not enough of the high protein fore milk.  Is
the baby being supplemented at all? (with EBM)  How is the weight gain?  All
these things are so hard to determine over the NET.
These little ones can sometimes do a great job at BF at the beginning, then
have setbacks and problems, OR, they can do poorly at breast and need lots
of time and patience to finally "get the hang of it".
Refer her to someone who can see what is reaally going on.

Just a note on my client with the 24 wk twins...The funeral for the baby who
died was really sweet.  As the priest stepped up to give the Eulogy I looked
up at the sky wondering (for the umpteenth time) why such a tiny baby had to
be takes from her mommy so early (both from the womb and the Earth).  What I
saw was amazing, to me.  There was a large flock of white birds, I don't
know what type.  They were flying in formations above the cemetary.  They
stayed until the service was over, then flew away.  IT was eerie and at the
same time reasurring, you know?  Powerful...

Also, mom went to the hosp Wed and told them no more ABM for her remaining
baby.  Baby is to have ONLY EBM.  The hosp is still diluting it, but I have
gotten some good advice/references from some of you out there (Thank You!)
so maybe that will help.
