Does anyone know of a good source for a form to submit to insurance for private practice services from an IBCLC?
It has been awhile since I have used these forms and I need to make sure it is up to date with the current codes and requirements.

On a positive note an insurance just referred a client to me for services.  They told her I was the closest IBCLC to her location.
Does anyone know if insurance will reimburse for travel fees if the client lives 45 miles from me.
