What a waste to throw away milk that the baby has left.  Where is the
evidence that this is a problem?  It's not like we are talking about 28 week
premature babies. Breastmilk is full of antiinfective agents.  Whatever
infective agents which will have gotten into the milk from the baby's mouth,
after all, came from the baby's mouth, no?  Milk doesn't spoil in so short a
time as 3 or 4 hours.
