Hi Nikki,
"breast rest" might be helpful but if exclusively pumping/expressing  mom
might still experience some pain and discomfort. What about using a  nipple
shield to protect the nipples during feeding? Is mom using hydrogels for  comfort
and healing between feedings or breast shells to help avoid discomfort  from
clothing?

If she decides on "breast rest" a good autocyling pump may be more
comfortable than hand expression. How old is baby? I have worked with a few  tongue tie
issues in the last few years and each one was unique. But from what  you
describe, this lady needs to rest and heal those nipples, get baby's  frenulum
clipped and get him back to breast. If they are concerned about nipple
confusion, a cup or finger feeding can be used for this brief time. One of the  cases I
worked with comes to mind: baby was several months old, mom had suffered  for
that whole time and she finally found a pediatric dentist who would clip.
Almost everyone I've worked with has described the latch as "night and day"
before and after the clipping and usually much more comfortable.
