I do not know of any öfficial"evidence based practice guidelines, but logical thinking comes with this:
Evidence shows that lactating breast have to be emptied in a regular pattern, not exceeding 3-4 hours
Evidence shows that not emptied breast will cause symptoms of illness such as discomfort, inflammation and possibly fever
Evidence shows that a surgical patients will do best if all vital function are functioning well and patient is not in fever or having inflammations.

Combining these I'd say it is best practice to see that somehow the breasts of a lactating surgical patients are being emptied at intervals of maximum 3-4 hours after starting the surgical procedure with as empty breasts as possible.

Warmly,

Do you have any evidenced based practice recommendations or other for the
lactating/breastfeeding surgical patient during an extensive surgery? I am
looking for information specifically for the perioperative period- during
the surgery- which could extend beyond four hours. Does anyone recall this
topic (in general, the breastfeeding surgical patient) in any of our
evidenced based practice resources. I have completed an extensive search, to
no avail. Thank you so much and I love our ability to connect and support
each other through Lactnet.
