Barbara, I think this "lactation nurse" is in for a shock. Having prepared
a packet for the Baby Friendly people, I can tell you there is no ambiguity
about the kinds of things you are saying this hospital does. They will not
get this status without a serious overhaul!

You might be able to implement something really neat there once they
realize that they DO need your help!

I would ask "Why do you want to be Baby Friendly?" They may be under some
pressure to do this and the pressure may be coming from a major benefactor
or something. If that is the case they are going to take it very seriously.

Your mission...should you decide to accept...could be to do the job of
three LC's with the salary of one. That's typical though, we all do it!
