Well, I spent 2 and a half hours with the mom yesterday.  Baby was
NOT latching on properly, only sucking on the nipple, not
compressing the sinuses.  Had mom pump with Medela Classic for 10
minutes and only got 20 cc from breast that had not been nursed
for 3 hours.  Babys weight was the same as the day before in the
doctors office.  Instructed mom to feed baby every 1.5 to 2 hours
for 15-20 minutes sucking time per breast and then to pump for 10
min with double pump ( mom rented Lactina select ).  Instructed
mom how to syringe feed baby any milk she may get, but warned mom
that she may not get anything. Also had mom watch short video on
positioning.  Mom was instructed to watch several BF videos while
she was in the hospital, but elected not to at that time.I had
worked with this mom in the hospital and baby was latching
correctly then.  This moms hubby is supportive but she really has
no friends or family in the area.  She says "I am so in love with
my baby."  Its so nice to hear a mom say that with the glowing
look she has on her face.  She really, really wants this to work.

Mom brought me a dirty diaper that her daughter had  several hours
before coming to see me, and it appeared to be a normal breastmilk
stool.  Mom states 5-6 mod wet diapers per 24 hours.  She is using
cloth.

Surgeon referral was made by Ped to rule out intestinal blockage,
per the mom.  Mom is not comfortable with this and I don't feel
its necessary.  Mom had ap pt with surgeon on Mon but says she's
going to postpone it and see how things go over the next week or
so.  I think baby is not stooping because she's not getting enough
milk....   no surgeon need for that!


Mom is Pourito Rican, but is an RN here in the states, her English
is good but does have an accent that makes communication a slight
problem. Mom had the sheet by LaLeche titled How to tell if my
healthy full term infant is getting enough milk, she also has a
form I give that has about 10 question to ask yourself when baby
is 4-5 days old and gives info on when to call for help.

Mom saw ped this am (Fri) and he called me and she is coming in
this afternoon for SNS.  I wanted to start it yesterday, but ped
and mom elected to wait another day.

I will update you all after I see her this afternoon.  THANKS to
all of you who relpied to my plea for quick help ! Its nice to
know that I have so many helpful Lactnet Collegues! :)

How do you all determine the amount of formula to put in the SNS
and how often do you tell moms to use it?  Just wondering what
other ppl do?  Thanks
