As far as I understand it, KMC requires the  "kangaroo" position which is 
vertical  - sternum to sternum - to stimulate the Vagus nerves in mother and 
baby.  The ventral surface is important - I seem to recall that babies tied 
to mother's backs did not produce the same results.  But any skin-to-skin 
contact is surely valuable?
