I think I would ask this mother to keep a log of feedings (length, which breast,
times, wet and messy diapers, supplements; how much and when) for 48 hours.  I have
had several instances when a mom thought she was feeding every 2 1/2 to 3 hrs but
when put in a log it was found that she may only be feeding every 4 to 5 hrs.

Is the baby nursing at night?  Sometimes a baby that will gain well with just 6
ounces of formula a day is going to long at night between feeds and all that is
needed is to pick up one or two feedings here.

Does baby go longer between feeds during the day when there is supplementation?
Mother may be trying to stick to this chacteristic feeding pattern of an infant
taking formula rather than increasing the breastfeedings as she comes off of the
supplement.

One other thing that I have run in to in the last couple of years are moms who are
on very low fat or no fat diets (ahh the wave of the 90's) after suggesting that
mom try to increase levels of fat in her diet baby's weight problem turned around.
