I am helping a mother who has gotten through bilateral MRSA+ abscesses and narrowly avoided surgery.  Obviously, she is very anxious to avoid further blockages and mastitis.

She finds her baby sleeps longer at night than the breasts would like, and she frequently wakes after 4 to 4.5 hours feeling very full, but the baby is still in deep sleep and if she attempts to feed baby at this point, it is ineffective.  She is currently expressing to comfort at that point.

I am surprised I have not run into this before, but perhaps most mothers would just wait it out, or express a little without worrying.
Have you worked with mothers who's milk supply never did match up with the baby's needs?  Is 9weeks still within normal time frame for night time too much milk?

Baby does have a Class III lip tie and mild posterior tongue tie, and the parents are deciding whether to have them clipped.  The baby does feed fairly well and there is now no nipple damage, pain or distortion.
