I recently supported a mother with overactive let-down and a colicy baby who had green, explosive stools. What finally worked for this mother was block feedings on one side for 8 - 10 hours for two weeks! Then, once her baby was breastfeeding more comfortably, and his poops turned to creamy yellow, she backed off the blocks to 4 - 6 hours, which she still maintains. Her baby was born in July. (And I have her permission to share this information.)

I've found that overactive let-down/oversupply issues sometimes take many weeks (and a lot of work on the mother's part) before the supply and demand system is back into a healthy balance.

Take Care,

Subject: Overactive let down reflex

I saw a 30 year old P1L1 on Sept. 1. Atypical let down from what i've ever
seen. There is and was no engorgement, nor fullness between feeds yet the
let down is so forceful that baby is in acute distress at every feed. Hungry all
the time-can't fill her up. Baby is 2 weeks old and transferred .7 oz from
breast in 1 minute!!! Tried some different positioning. Tried to do one sided
nursing for between 4-6 hour stretches and Mom never felt any fullness or
need to pump to comfort on other side. Talked to mom by phone. No
improvement. Feeding almost every hour. Pumped yesterday 3 oz. after
feeding. No hx of galactorhea. No leaking prenatally or now except when
feeding on one side. FORCEFUL let down almost immediately post latch.
Does anyone have suggestions on how to proceed? Need help asap!!
