have to agree totally.  Even if you were to change the diaper immediately,
think of the force of bowel movements with most breastfed babies.  My girls
diapers had blow outs more times than I care to remember.  If the diaper is
fresh it's going to be a little loose in the crotch area (not puffed
up-hard to describe) and those poops are going to have a lot of room to get
around, If it can go up and out the front *and* the back, you have to
imagine it can go up and into the urinary tract as well.  Of course
changing frequently and cleaning correctly is going to reduce the chances,
but not eliminate them.
