Jay another consideration may be to look at the palate shape of the infant
or how the baby is sucking.  I have no doubt that your positioning  and
latch is perfect, so I would next look for what the baby may be doing in
his mouth.  I have had several clients with very severely damaged nipples,
usually from unusual palate shapes, that sound like your client.  Other
than trying the baby at the breast infrequently and seeing what happens I
found that most of them were unable to exclusively breastfeed (at the
breast) until around 8 weeks or 5kg.  This seems to be the size when the
baby can take the breast further back into his mouth past the unusual palate.
Kathleen and Pamela that is really interesting about a really nasty
infection causing this too.  I'll keep that knowledge up my sleeve
too.  Pamela did you swab the nipple or culture the milk in the cases you
dealt with?
