It seems that the answer to this mother's dilemma is in the last statement.  "After 3 pm and until morning she usually pumps to give her breasts a break".  If pumping doesn't hurt her nipples it is because she can position the flange properly but if breastfeeding is painful it's likely because the latch is not right. (And lots of bottles can also contribute to the poor latch / way the baby sucks).

 I see a fair amount of what the doctors diagnose as Reynauld's but once the latch is fixed, no more Reynauld's.  I refer to this as a "Reynauld's like syndrome" caused by nipple trauma and reassure the mother she doesn't have a "disease".  From my experience,  If it is true Reynauld's, it will also be very painful to pump.
