Anne,
I think the best thing for your sister to do would be attend LLL
meetings while still pregnant, read about bf from some good sources like
The Womanly Art, encourage an unmedicated birth, or at the very least
not an induction (soooo common nowadays), and focus on the normal and
best early bf practices. She may meet a large breasted mom at LLL. This
does not necessarily predict problems, of course, it can combined with
birth meds, early suppl, etc. Of course this would negatively impact a
small breasted woman as well.
Lactnet is not the place for her, as it is meant for lactation
professionals. If there is a good IBCLC in the hosp where she will
deliver (I'm assuming hosp birth) have her make a prenatal appt for a
consult. I have talked to lots of these worried moms and they have so
many questions and misunderstandings  and myths that we easlily clear
up. Like one recently was going to bf x 6 wks and after talking to me
says now she will go for 1 yr. I think I had assuaged all her fears and
blasted alot of the "rules" about bf (diet, etoh, etc). Best of luck to
