Unfortunately, there is no "research" evidence about treatment of 
nipple/breast thrush.

We say that baby should be treated if the mother has nipple/breast thrush, 
so the infection does not ping-pong back and forward. But if only the baby 
has oral thrush and mother has no symptoms - then treating baby only is fine 
(just warn the mother what to look out for).

I find that clotrimazole doesn't work as well on the skin as it does for 
vaginal infections. We use miconazole (Daktarin ) in Australia (gel for 
baby, gel or cream for mother).  I know you don't have the Daktarin gel in 
the US, but do you have the cream?
It is not possible to comment further without a full history - some women 
might respond to 2 fluconazole, others (eg long history of vaginal thrush, 
recent antibiotics, etc) may need more.
