Subject: Gelatinous Milk!

Hoping those with vast years of experience might be able to identify an unusual phenomenon for me. PTP! In fact, client is begging for answers.

Working with first time mom who started out breastfeeding her baby well until baby readmitted for jaundice just as her milk was coming in and was told to stop breast and pump only so they could "see" volume. Ended up requiring formula supplementation. Of course, difficulties latching since then. Baby gaining weight but having to supplement with formula at night. Mom reports difficult to get on breast, does 10 mins well then very angry and won't get back on either side. Has to f/u with bottle. Mom reports he's a "messy" bottle feeder but . Saw at 2 weeks for first time. Did 20/10 with signs of deep latch after minor adjustments and without fussiness. No signs of oral restriction. Poor suck/swallow ratio on second side. Breast compressions helped minimally. Interestingly, transferred 1/2 oz each side. Mom can hand express easily. Pumps about 4 oz total but here's the kicker: for previous 2 days has had to strain out large gelatinous clump (usually lightly pink tinged) which cuts her usable volume by 1/3 to 1/2!. I've seen clumpy or stringy milk before but not this consistency! Looks like large clots or several raw eggs when poured from one bottle to another. Comes from both sides. Can message/text video if anyone wants to see. Actually clogs her pump where she has to stop and get them out! No signs of clogged ducts or mastitis. Has been to OB to make sure there is no infection. Of course, doc told her to stop putting to breast in case baby chokes on globs. I saw no indication of this during our visit. Can't imagine the baby being able to get this out. Not sure how they even fit thru the nipple pores!!! If mom hadn't reported having to stop and clear the tunnel to be able to continue, I would have thought it was clumping once it got in the bottle. This is still continuing 5 days later. Mom believes it got worse when she did more breastfeeding because baby "wasn't getting them out". Mom has backed off on the vacuum of pump to rule out damage from suction.

1. Wonder if the baby suckles well until one of these "clots" gets to the exit at which point baby gets frustrated or tired of trying to get milk out
2. If there is ductal bleeding, could a blood clot be incorporating milk within the clot?
