"Confirmed not pregnant, and not on meds" .. but is there perhaps an IUD
with some low levels of progestin, or was there a Depo shot in there?

Pumping for 30 mins at a pop seems like too much -- I would tell her to use
the Hands On Pumping techniques developed and studied by Jane Morton MD:
http://www.leron-line.com/updates/Hands_onPumping.pdf.  Admittedly this is
not a premie (Morton works in a NICU) but the technique of breast massage in
addition to the mechancial pump does seem to boost supply.
