The price set includes the MD time, effort, skill as well as  office 
overhead, malpractice insurance, equipment cleaning/sterilization,  assistant 
time, etc. - all of which vary greatly from one area of the  country to the 
other.  Every MD I know will "eat" the cost in hardship  cases.
