Girls, I find it really difficult to assess whether a baby has a posterior 
tongue tie... :-(
Yesterday I had a consultation with a family; baby still little, not really 
very small (3230 grams), but somehow looking tender and small anyway.
It's a 42 weeker, so I even wondered in the back of my mind whether a little 
dismaturity might be the case.
Okay, we practised with latch; nipple had a sharp edge/ridge when coming out 
of baby's mouth and that was exactly where a few sores were.
Had a consultation the day before and checked both babies' palates. Second 
one was higher, but I don't know the width of normal variation without 
problems yet.
Football hold did seem to work a little better, indeed, with the higer 
palate. But was it a baby problem...? Could be a match-problem: small baby 
and really protruding nipples.
I think if the baby will grow, the problem will solve itself, as there was 
already an improving tendency.
Baby could stick out her tongue quite well, had free range of motion, no 
asymmetrical movements, no humping or thrusting.
So... really... how to assess whether this is tongue-tie?! All looked well!
I really wonder... should we clip all those babies or should we realise that 
separating them from their mothers and suboptimal or sometimes even really 
bad bf techniques cause damage in the developing relationship, that can only 
be solved with patience and closeness and tender care and support for mom 
who should know she is doing the right thing by feeding véééry often...?
I know that many of you are wildly enthusiastic about discovering and 
treating tt, but I still have doubts as to where the limit should be.
We should all be very aware of the value of the process and not overrule it 
with all our technical wisdom. As Kathy Kendall says: "Easy breastfeeding in 
387 steps" and I add: "Your lc knows them all!" ;o)
