UNICEF in the UK has a seven point Baby Friendly community programme, Ruth.

We have exactly the same issues here.

You can bump up your initiation rate and bf on discharge,  but it
leads to a lot of heartaches and disappointment when bf is not
sustained because of poor support outside the hospital, with
incorrect and conflicting info.
