Jack,
I saw a primip with serious engorgement at about 20 weeks gestation.  I gave
her instructions on use of cabbage 30-45minutes 1-2 times per day.  She
followed those instructions for 3 days, then said the problem resolved.  When
her baby was born, she had a full and plentiful milk supply.

Let us know what happens in this case.
