I would put my money on a tongue-tie.   Take your finger and gently lift the 
tongue from underneath...if you cannot see it, you should still be able to 
feel it.

The other possibility is baby is waiting for a stiff stimulus in the roof of 
the mouth like the bottle/fingers he is used to.  You could try a nipple 
shield and see if he gets going with that.  It could also help with a 
tongue-tied baby...but weight must be followed for 2 months as they often 
cannot remove milk well.
