Absolutely I have gotten 3 mo old babies and older!  to the breast who have 
NEVER breastfed with shield or anything else.  But there needs to be a milk 
supply, and it sounds like this mom doesn't have much.  that's the first thing.  
the baby needs a reason to go there!  Also, i find it a little strange that this 
baby is feeding lying on his back by himself, w/o cuddling.  I think there's more 
going on here than just latch issues.  I have NEVER met a baby, capable of 
grasping the breast and staying there, who would not breastfeed given the 
chance and given a full milk supply.  I've been in private practice for 11 years 
and a LLLeader for 16 years.  the oldest baby i worked with getting on the 
breast was 7 months old.  mom had been pumping the whole time because 
baby was born with a cleft palate.  after it was repaired at 4 mo, baby still 
wouldn't nurse.  She finally called me at 7 mo.  we started with a shield, with 
the baby half asleep and shortly after that, the babe was completely 
breastfeeding!  Let me know what happens!
