I'm confused by the phrasing. I tell moms to have the baby finish the first breast first, then offer the second breast -I liken it to dessert, sometimes you'll want dessert, other times you won't. If baby is going through a growth spurt, he'll take the second breast and the supply will be upgraded. The second breast is "offered" but it's up to the baby whether both are emptied. I think that the most important part of this discussion is the baby, and following his lead.
