If I might clarify on Cathy's comment about what I've noted on PCOS and milk
supply problems, the 30% problem rate came from a small convenience sample
only. The formal research has not yet been done to officially quantify the
true rate of problems within the population of mothers with PCOS, something
I would love to do. Within that 30%, one third of those had severe milk
deficits, while the others had moderate deficits. This just gives us an idea
of what may be going on, a head's up, let's-pay-closer-attention-from-now-on
kind of thing. When a woman presents with possible milk production problems
it is important to note her history, but also to look for all the other
possible problems and not to jump to conclusions too soon, either, as Cathy
wisely pointed out. How much I say to a mother depends upon how many clues
there are. If she has very little breast tissue I will say more up front; if
there are a lot of other possible factors, I will say less and play those
other cards first. With all of our current birthing practices and the myriad
of possible causes of delayed lactogenesis these days, I'm often not certain
of whether I'm dealing with a delayed milk issue or a true primary problem,
and usually only time will tell, so it is often fair to encourage a mother
to give her body a chance to recuperate and bounce back. I just try to paint
a fair picture for her, be as optimistic as is reasonable, and wait it out
with her with encouragement.
