Karyn-Grace asked for ideas for a surrogate mother who is pumping exclusively, who has a number of pumping issues, nipple trauma, and evidence of Raynaud's.  
 
I am wondering if one of the issues is referred pain?  I've seen this in a number of mothers where there is a history of nipple pain that is being addressed, but where the pain - when further investigated - is within the breast and very general.  In these cases, where other causes have been addressed, I ask the mother if she has a history of spinal injury, perhaps in a car accident.  The answer is usually "yes".  I then urge the mother to visit whoever was managing her pain or rehabilitation from the old injury, to have the pain treated, and then to come back to me once on medication.  If she has previously been seeing a physiotherapist (PT) for the old back injury, I enourage her to see the PT again.  I have found that asking this question can lead to new possibilities in helping the mother.  Even if the injury is higher than the particular thoracic vertabrae where the innervation to the breast comes off the spine, there may still be an effect (for reasons I'm not sure of).!
   The PTs on this list may be able to explain this.
