Sorry to seem TOTALLY naive, but why couldn't a mom advised by her doctor
to pump and dump following surgery, and who is educated to the contrary
by her LC and/or medical literature, just ignore the doctor's advice?
She would take any risk on her own shoulders, naturally, and would need
to make an educated decision, but isn't it her business, not his?  The
baby's not his patient.  I often feel angry when I hear
so many of you talk about doctors who insist that YOU prove beyond a
shadow of a doubt that bf'ing is OK in a given situation, ignoring the
known dangers of ABM, maternal-infant detachment, risk of breast health
problems (long and short term) to mom.  Who made them G_d?
